ABSTRACT

Aim: To evaluate the surface roughness and color stability of bulk-fill resin composites after simulated toothbrushing with whitening dentifrices. The radioactive/relative dentin abrasion (RDA) and radioactive/relative enamel abrasion (REA) of dentifrices were also assessed. Methods: Specimens (n=10) of Tetric N Ceram Bulk Fill (TNCB), Filtek One Bulk Fill (FOB) resin composites, and Z100(Control) were prepared using a cylindrical Teflon matrix. Surface roughness (Ra, µm) was assessed by a roughness meter and the color evaluations (ΔEab , ΔE00 , WID ) were performed using a digital spectrophotometer based on the CIELAB system. Three measurements were performed per sample, before and after simulated toothbrushing with 3D Oral-B White Perfection (3DW) and Black is White (BW) dentifrices. The abrasivity (REA and RDA values) of the used dentifrices was also determined by the Hefferren abrasivity test. Results: The Ra values increased significantly in all resin composites after 3DW and BW toothbrushing. The acceptable threshold color varied among resin composites, and TNCB and Z100 presented the highest ΔEab and ΔE00 for BW dentifrice. The 3DW dentifrice was significantly more abrasive than BW dentifrice on enamel and dentin. Conclusions: simulated toothbrushing with tested whitening dentifrices increased the surface roughness at acceptable levels. The Tetric N Ceram Bulk-fill and Z100 composite showed the highest color alteration in BW. 3D White Perfection dentifrice was more abrasive on dentin and enamel than Black is White.

ABSTRACT

Abstract This systematic review and meta-analysis aimed to analyze the impact of the COVID-19 pandemic on dental trauma patient attendance. The study was registered in the PROSPERO system, using the CRD42021288398 protocol. Searching was performed in PubMed, Scopus, Web of Science, Embase, Lilacs, and OpenGrey databases, using the following keywords: "Tooth injuries," "Dental trauma," "Traumatic Dental injury," and "COVID-19". We included observational studies evaluating dental trauma in the context of the COVID-19 pandemic. Quality assessment was performed using the Joanna Briggs Institute Critical Appraisal Checklist for Cross-Sectional Studies. Meta-analysis was performed in RevMan 5.4 software with Odds Ratios as a pooled measure of effect, with a 95% confidence interval, and using random-effects modeling. After applying the eligibility criteria, 32 studies were included for qualitative analysis, in which 10 were used to assess the frequency of dental trauma diagnoses in dental emergencies. Despite the decrease of visits during COVID-19, the analysis revealed no difference between the pandemic and pre-pandemic periods. Regarding the type of dental trauma, two of the studies revealed no differences for the periods before and during the pandemic. This study revealed that the COVID-19 pandemic has not impacted the frequency or type of dental trauma compared to previous periods.

ABSTRACT

Abstract: The objective of this review is to identify preventive measures for COVID-19 to safeguard dental professionals providing prosthodontic dental care. Electronic searches were performed in PubMed, Scopus, and Cochrane databases using the following descriptors and/or words: "COVID-19," "SARS-CoV-2," "Dental practice," "Prosthodontics," and "Dental infection control." Prosthodontists are at high risk for exposure to the novel coronavirus through aerosols and possibly contaminated surfaces and indirect contact with dental laboratories and dental technicians through impressions, dental stone casts, and fixed and removable prosthetic appliances. Therefore, preventive measures should be implemented, including performing emergency treatments only, rescheduling patients with suspected disease, disinfecting surfaces and prosthodontic materials with biocidal substances, and using protective equipment such as N95 masks, disposable gowns, and face-shields.

ABSTRACT

Abstract Objective: To investigate the impact of inadequate prostheses on older adult's quality of life at the Integrated Health Center for Older Adults in the northeastern city of Brazil. Material and Methods: A cross-sectional study involving 147 older adults aged 60-90 years old in João Pessoa, Brazil. Interviews and oral examinations were carried out to collect demographic characteristics (age and gender) and clinical dental measures (use of inadequate upper and/or lower dental prosthesis) and Oral Health Impact Profile (OHIP-14 Questionnaire). The Pearson Chi-square and Fisher's exact tests were used to evaluate the association between the OHIP-14 dimensions and the use of unsatisfactory prostheses (p<0.05). Results: The sample consisted of 90.5% of women being a mean age of 69.86 (± 6.60) years. Individuals with inadequate prostheses had a higher impact on quality of life, with a predominant prevalence ratio for developing physical pain (PRR = 3.52; CI = 4.69-2.65), psychological discomfort (PRR = 5.0; CI = 5.85-4.26) and psychological inability (PRR = 1.71; CI = 2.56-1.14). Conclusion: The use of inadequate prostheses had a negative impact on the quality of life of the older adult population. Therefore, services, which provide health care for older adults, should overcome this reality by providing broad access to oral rehabilitation that is appropriate to the needs of each individual.

ABSTRACT

Abstract We aimed to evaluate the orofacial antinociceptive effect of geraniol in mice and its molecular anchorage mechanism. Seven mice per group (probabilistic sample) were treated with geraniol (12.5, 25 and 50 mg/kg, i.p.), morphine (6 mg/kg, i.p.) and vehicle (saline + Tween 80 at 0.2%, i.p.) 30 minutes prior to the beginning of the experiment. Injecting glutamate (25 μM), capsaicin (2.5 μg) and formalin (2%) into the right upper lip (perinasal) of the mouse induced nociception. Behavioral analysis of the animals considered the friction time (in seconds) of the mentioned region using hind or front paws by a researcher blinded to the treatment groups. The statistical analysis was performed blindly, considering α = 5%. The results showed that in the glutamate and capsaicin tests, concentrations of 25 mg/kg and 50 mg/kg presented antinociceptive activity (p < 0.005, power> 80%). In the formalin test, geraniol was able to reduce nociception at a concentration of 50 mg/kg (p < 0.005, power> 80%). In the molecular anchorage study, high values of binding between the evaluated substance and receptors of glutamate were observed (metabotropic glutamate receptor, -87.8501 Kcal/mol; N-methyl-D-aspartate, -86.4451 Kcal/mol; α-amino-3-hydroxy-5-methyl-4-isoxazole propionic acid, -85.6755 Kcal/mol). Geraniol presented orofacial antinociceptive activity, probably by interacting with glutamate-related receptors.

ABSTRACT

Abstract This study aimed to explore the association between use and need of dental prostheses to the quality of life in elderly individuals in a Referral Center for Elderly Care in the northeastern of Brazil. A cross-sectional study was developed with 199 elderlies of both sexes. Data were collected from clinical examinations (WHO criteria) and a questionnaire regarding socioeconomic status, Oral Health Impact Profile index (OHIP-14) by a single calibrated examiner with Kappa = 0.91. Data were analyzed using Mann-Whitney test, Poisson regression (p<0,05). The sample consisted of 84.9% women, mean age of 70.02 (sd±6.50) years. There was an independent association between quality of life and the covariates sex and need of dental prostheses. Men showed lower values of the score (PR = 0.734, p = 0.011) than women. Individuals without normative need for dental prostheses showed lower values of OHIP-14 (PR = 0.767; p = 0.003) than those in need.


Resumo O objetivo desse estudo foi verificar a associação entre o uso e a necessidade de prótese dentária e qualidade de vida em idosos de um Centro de Referência de Atenção ao Idoso no Nordeste do Brasil. Um estudo transversal foi desenvolvido com a participação de 199 idosos de ambos os sexos. Os dados clínicos foram coletados conforme a Organização Mundial de Saúde, através de exames clínicos e questionário sobre dados socioeconômicos, índice Oral Health Impact Profile (OHIP-14), por um examinador previamente treinado (kappa=0,91). Os dados foram analisados pelo Mann-Whitney, regressão de Poisson (p>0,05). A amostra consistiu de 84,9% de mulheres, com idade média de 70,02 (dp±6,50) anos. Houve associação independente entre a qualidade de vida e as covariáveis sexo e necessidade de prótese. Idosos do sexo masculino apresentaram menores valores do escore (RP=0,734; p=0,011) do que as mulheres. Indivíduos sem necessidade normativa de prótese dentária apresentaram menores valores do OHIP-14 (RP=0,767; p=0,003) do que aqueles com necessidade.

ABSTRACT

ABSTRACT Purpose: to evaluate the methodological quality of clinical trials published in Brazilian journals. Methods: four trained independent researchers conducted a systematic literature search of all Brazilian speech therapy-related journals over the last ten years, whether the journals were active or inactive. All journal volumes published during that period were selected, and each researcher conducted an individual analysis to identify articles that focused primarily on orofacial motricity. The tools used were the Downs and Black Quality Checklist and the Jadad scale. Results: after the studies were selected and categorized, the final sample comprised six articles, all of which were classified as clinical trials. The observed methodological limitations included a lack of sample planning, randomization and blinding. Mean scores of 16.3 points on the Downs and Black Quality Checklist and 2.3 on the Jadad scale were obtained. Conclusion: the randomized controlled trials in the area of orofacial motricity are scarce in Brazilian literature, suggesting that studies in this area adopting this research design should be expanded and their quality should be improved to promote clinical practice based on scientific evidence.


RESUMO Objetivo: objetivou-se avaliar a qualidade metodológica dos ensaios clínicos publicados em periódicos brasileiros. Métodos: trata-se de uma pesquisa bibliográfica sistemática. Participaram quatro pesquisadores independentes treinados que realizaram um levantamento dos últimos 10 anos em todos os periódicos nacionais da área da Fonoaudiologia, ativos ou inativos. Para tanto, foram selecionados todos os volumes desse período de tempo e cada pesquisador realizou análise individual a fim de identificar estudos que tinham o principal objeto de estudo enquadrado na Motricidade Orofacial. Foram utilizados os seguintes instrumentos Downs and Black Quality Checklist e escala de Jadad. Resultados: após a seleção e categorização dos estudos, a amostra final desta pesquisa foi composta por 6 artigos classificados como ensaios clínicos. Dentre as limitações metodológicas observadas destacam-se a ausência de plano amostral, randomização e cegamento. Identificou-se uma média de 16,3 pontos para o Downs and Black Quality Checklist e 2,3 para escala de Jadad. Conclusão: conclui-se que a literatura brasileira é escassa de ensaios clínicos controlados randomizados na área de motricidade orofacial, sugerindo ampliação e qualificação das investigações que adotam esse delineamento de pesquisa, promovendo, dessa forma, uma prática clínica baseada em evidência científica.

Resumo O objetivo deste estudo foi avaliar a prevalência de sinais e sintomas de disfunção temporomandibular (DTM), sua associação com gênero, hábitos parafuncionais, tensão emocional, ansiedade e depressão e, o seu impacto sobre a qualidade de vida relacionada com a saúde oral (QVRSO) em estudantes pré-vestibulandos de instituições públicas e privadas de João Pessoa/PB. A presença de sintomas de DTM foi determinada através de questionário anamnésico, contendo também questões relacionadas à presença de hábitos parafuncionais e tensão emocional. Um protocolo simplificado de avaliação clínica foi aplicado. Ansiedade e depressão foram determinadas através da escala Hospital Anxiety and Depression (HAD) e, a QVRSO avaliada por meio da versão resumida do Oral Health Impact Profile (OHIP-14). Foram realizados testes de Qui-quadrado, Exato de Fisher, Mann Whitney e Kruskal-Wallis. A presença de sinais e sintomas de DTM foi estatisticamente associada ao gênero feminino, hábitos parafuncionais, tensão emocional e ansiedade, e representou maior comprometimento da QVRSO. A elevada prevalência de sinais e sintomas de DTM entre os estudantes pré-vestibulandos sinaliza a necessidade de divulgação e esclarecimento de professores e alunos visando o diagnóstico precoce e a prevenção.


Abstract The aim of this study was to evaluate the prevalence of temporomandibular disorders (TMD) signs and symptoms, its correlation with gender, parafunctional habits, emotional stress, anxiety, and depression and its impact on oral health-related quality of life (OHRQL) in college preparatory students at public and private institutions in João Pessoa, Paraíba (PB). The sample consisted of 303 students. Presence of TMD symptoms was determined by an anamnesis questionnaire containing questions related to the presence of parafunctional habits and emotional stress. A simplified clinical evaluation protocol was used. Anxiety and depression were determined with the Hospital Anxiety and Depression (HAD) scale and the OHRQL using the short version contained in the Oral Health Impact Profile (OHIP-14). The Chi-square, Fisher Exact, Mann Whitney, and Kruskal-Wallis tests were performed. Presence of signs and symptoms of TMD was statistically associated (p ≤ 0,05) with female gender, parafunctional habits, emotional stress, and anxiety, and represented greater impairment of the OHRQL. The physical pain domain was the most affected. The increased prevalence of signs and symptoms of TMD among college preparatory students indicates that there is a need for education and clarification among teachers and students to improve early diagnosis and to prevent the problem.

ABSTRACT

Verificar as características, a extensão e a profundidade da lesão, e a hipersensibilidade das lesões cervicais não cariosas (LCNC) em pacientes com alterações oclusais. Sujeitos e método: por meio de um estudo transversal e observacional foram avaliados 58 pacientes com lesões, de ambos os sexos; entre 18 e 71 anos, nos Serviços de Oclusão das cidades de João Pessoa e Campina Grande-PB. Extensão, profundidade e hipersensibilidade de LCNC foram determinadas por exame clínico realizado por único examinador calibrado. A análise dos dados foi realizada por meio de análises descritivas e não paramétricas (coeficiente ρ de Spearman) com 5% de nível significância e 95% de intervalo de confiança. Resultados: extensão supragengival com índice de desgaste dental de grau 1 foi prevalente na população estudada. A idade apresentou correlação positiva com o aumento da profundidade das lesões. A hipersensibilidade cervical esteve presente em 74,13% e mais prevalente nos pré-molares inferiores (22,34%). Conclusão: Lesões com extensão supragengival foram prevalentes entre os pacientes da pesquisa; pré-molares inferiores foram o grupo dental mais acometido pela hipersensibilidade e o aumento da profundidade das lesões esteve associado a presença da hipersensibilidade das LCNC.

RESUMO Esta pesquisa revisou de modo integral a produção científica brasileira em Motricidade Orofacial nos últimos 10 anos e teve como objetivo realizar uma descrição e categorização geral das publicações em motricidade orofacial e analisar os estudos quantitativos na área. Os dados foram coletados em todas as publicações em motricidade orofacial publicadas nos últimos dez anos, após uma análise integral de todos os periódicos brasileiros em Fonoaudiologia. Os artigos foram categorizados quanto ao ano, tipo de artigo e área temática. Além disso, foram analisados sobre a faixa etária, abrangência, essência do objetivo e tipo de estudo todos os artigos realizados com seres humanos e com dados quantitativos. Os dados foram discutidos sob a ótica da metodologia científica aplicada ao aperfeiçoamento do método e desenvolvimento da ciência baseado em evidências. Além disto, os dados são analisados com sugestões de aspectos potenciais a serem considerados para futuras pesquisas na área.


ABSTRACT A complete review of Brazilian scientific articles on orofacial myology (OM) published in the last 10 years was conducted to describe, categorize and quantitatively analyze studies in the field of OM. Data were collected from all Brazilian journals on speech-language and hearing sciences published between 2005 and 2015. All articles were reviewed and categorized according to the year of publication, type of article and subject area. Furthermore, all studies conducted on humans that reported quantitative data were analyzed regarding age group, scope, main objective and type of study. The data were discussed from the standpoint of scientific methodology applied for the improvement of methods and for evidence-based scientific development. Moreover, aspects to be considered in future studies in this field are suggested.

ABSTRACT Objective: Determine the prevalence of signs and symptoms of temporomandibular disorder (TMD) and its relationship with parafunctional habits in a sample of adolescents. Methods: The sample consisted of 129 high school students, between 16 and 19 years old in public schools. Data were collected through: a questionnaire about parafunctional habits and TMD symptoms, the Fonseca's index (DMF) and a summarized protocol of clinical evaluation of the TMD signals. These data were analyzed in a descriptive way, using statistical analysis by Chi-square test (x2) and Fisher's Exact test. Results: 84.5% of individuals showed a degree of auto-reported dysfunction. The most frequent parafunctional habit was chewing gum (65.9%) while the habit of biting objects was significantly associated with the presence of TMD symptoms (p = 0.042). There was a statistically significant association between TMD symptoms with variables such as gender (p = 0.032) habits (p = 0.014) and emotional stress (p <0.01), besides the association between the need of treatment and gender (p = 0.002) and stress (p <0.01). In addition, there was an association between muscle tenderness, unilateral mastication (p = 0.045) and chewing ice / lollipop (p = 0.04). Conclusion: It was concluded that there was not only a high incidence of signs and symptoms of temporomandibular dysfunction and parafunctional habits in high school students, but also an association among them, becoming necessary to establish public policies of prevention and access to treatment for these problems for that specific age group.

Métodos: A amostra foi constituída de 129 estudantes, de 16 a 19 anos de escolas públicas de ensino médio. Os dados foram coletados por meio de: um questionário sobre os hábitos parafuncionais e sintomas de disfunção temporomandibular, o Índice anamnésico DMF de Fonseca e um protocolo resumido de avaliação clínica de disfunção temporomandibular. Os dados foram analisados de forma descritiva, com a análise estatística realizada pelo Teste Qui-quadrado (x2) e Exato de Fisher. Resultados: Houve prevalência de 84,5% de indivíduos com algum grau de disfunção temporomandibular. O hábito parafuncional mais frequente foi o de mascar chicletes (65,9%) e o hábito de morder objetos teve associação significativa com a presença de disfunção temporomandibular (p=0,042) Houve associação estatisticamente significante entre sexo e disfunção temporomandibular (p=0,032), disfunção temporomandibular e hábitos (p=0,014), disfunção temporomandibular e tensão emocional (p<0,01) e tensão e necessidade de tratamento (p<0,01). Além disso, houve associação entre sensibilidade muscular e mastigação unilateral (p=0,045) e mastigação de gelo/pirulitos (p=0,04). Conclusão: Conclui-se que nos adolescentes do ensino médio houve alta prevalência de disfunção temporomandibular e hábitos parafuncionais, bem como a associação entre os dois, sendo necessário o estabelecimento de políticas públicas de prevenção e acesso a tratamento para esse problema e grupo etário específico.

Objective: To evaluate the prevalence of dentin defects, including partial and complete cracks and fractures, after root canal preparation in molars with Reciproc and WaveOne reciprocating instruments. Material and Methods: Fifty mandibular first and second molars with mature apices were selected to endodontic in vitro instrumentation. Ten teeth were unprepared and served as the control, and the remaining forty teeth were divided into two groups, being twenty corresponding to each reciprocating system. Reciproc and WaveOne systems were used in a reciprocating working motion, under the same conditions, to prepare the two mesial canals. Roots were then sectioned 2, 4 and 6 mm from the apex, and the cut surface was observed under a microscope using 20-fold magnification and checked for the presence of fractures and incomplete cracks. Results: none of the evaluated groups presented fractures, and the control group showed no incomplete cracks. Overall evaluation showed statistical differences between these two groups and the control group (p=0.017) for the prevalence of incomplete cracks, but no significant differences were obtained between Reciproc and WaveOne groups (p>0.05). Reciproc group presented more incomplete cracks on 4-mm sections compared with the control group on the same section (p=0.0326). Conclusion: Root canal preparation with both reciprocating instruments resulted in incomplete cracks, but not fractures. At the level of 4 mm from the apex of the canals, the Reciproc system produced significantly more incomplete cracks. Considering both systems, WaveOne utilization resulted in lesser structural alterations on dentin considering the middle portion of the roots.

Objective: To evaluate the occlusal aspects of patients with and without noncarious cervical injury and identify its risk factors. Method: Through a cross-sectional study, 88 patients of both sexes between 18 and 71 years old were evaluated, in Occlusion services in the cities of João Pessoa and Campina Grande. Clinical assessments were performed by a single calibrated examiner to check the occlusal aspects of patients with and without injuries, through descriptive and non-parametric analysis with 5% significance level and 95% of confidence interval. Results: Parafunctional habits were identified at a frequency of 92.0% of the patients. Unilateral chewing was not associated with the presence of lesions. The side disocclusion by the canine was prevalent in the whole sample. Occlusal interference in maximal intercuspal position and in nonworking side were presented as risk factors for the higher number of lesions and their development. Wear facets were common in the premolar groups and the maxillary arch in patients with lesions. Conclusion: Occlusal factors pose risks for the development and greater number of noncarious cervical lesions.


Objetivo: Observar os aspectos oclusais de pacientes com e sem lesão cervical não cariosa e identificar os fatores de risco das mesmas. Método: Através de um estudo transversal foram avaliados 88 pacientes, de ambos os sexos, entre 18 e 71 anos, nos Serviços de Oclusão das cidades de João Pessoa e Campina Grande. Realizaram-se avaliações clínicas, por um único examinador calibrado, para verificar os aspectos oclusais dos pacientes com e sem lesões. Através de análises descritivas e não paramétricas com 5% de nível significância e 95% de intervalo de confiança. Resultados: Os hábitos parafuncionais foram identificados em uma frequência de 92,0% dos pacientes. A mastigação unilateral não apresentou associação com a presença das lesões. A desoclusão lateral pelo canino foi prevalente em toda amostra. Interferências oclusais em máxima intercuspidação habitual e lado de não trabalho apresentaram-se como fatores de risco para o maior número de lesões e o desenvolvimento das mesmas. Facetas de desgaste foram frequentes no grupo de pré-molares e no arco superior em pacientes com lesões. Conclusão: Fatores oclusais representam risco para o desenvolvimento e maior número de lesões cervicais não cariosas.

Introduction: The relationship between dental occlusion and temporomandibular disorders (TMD) remains a subject of disagreement. Many professionals erroneously base diagnosis and treatment strictly on the occlusal factor, despite the fact that current scientific evidence does not show such a relationship. Objective: To evaluate the knowledge of dental surgeons (DSs) from João Pessoa (PB)-Brazil, regarding the relationship between occlusal factors and TMD. Materials and method: A sample of 100 DSs who do not have expertise in TMD and orofacial pain (CG Group) and seven DSs with this specialty (EG Group) completed a questionnaire that addresses issues concerning knowledge of TMD and its relationship with occlusal factors. The questionnaire also contained information used to characterize the sample, such as age, gender, and length of experience. The current literature's degree of consensus was established as the "gold standard" response for each statement and was compared with the responses of the specialists and non-specialists. Data were tabulated using the SPSS software package and analyzed descriptively (by percentage) and statistically using the chi-square and Fisher's exact tests (p < 0.05). Result: A wide divergence could be observed between the knowledge of DSs who do not specialize in TMD and orofacial pain and that of professionals who do. Conclusion: There was low agreement between specialists and non-specialists. The relationship between dental occlusion and TMD remains unclear for the vast majority of participating professionals, which may prove to be reflected in diagnostic behaviors and inappropriate occlusal treatment for the management of TMD.


Introdução: A relação da oclusão dentária com a Disfunção Temporomandibular (DTM) ainda gera divergências. Muitos profissionais erroneamente baseiam o diagnóstico e a terapia estritamente no fator oclusal, apesar das evidências científicas atuais não mostrarem tal relação. Objetivo: Avaliar o conhecimento dos cirurgiões-dentistas (CDs) de João Pessoa-PB, acerca da relação entre fatores oclusais e DTM. Material e método: Uma amostra de 100 CDs que não possuem especialização em DTM e Dor Orofacial (Grupo GN), além de sete CDs com essa especialidade (Grupo GE) preencheram questionário abordando assuntos relativos ao conhecimento da DTM e sua relação com fatores oclusais. O questionário continha também informações para caracterização da amostra como idade, gênero e tempo de formado. O grau de consenso da literatura atual para cada sentença foi estabelecido como "padrão-ouro" de resposta, o qual foi comparado com as respostas dos especialistas e não especialistas. Os dados foram tabulados no programa SPSS e analisados de forma descritiva (através de porcentagens) e, estatística, por meio dos testes Qui-quadrado e Exato de Fisher (p<0,05). Resultado: Observou-se grande divergência no conhecimento dos cirurgiões-dentistas sem especialidade em DTM e Dor Orofacial com os profissionais dessa especialidade. Conclusão: Houve baixa concordância entre os especialistas e não especialistas. A relação entre oclusão dentária e disfunção temporomandibular ainda permanece obscura para a grande maioria dos profissionais entrevistados, o que pode vir a se refletir em condutas de diagnóstico e tratamento oclusal inadequadas para o manejo das DTM.

ABSTRACT

Introdução: O papel da oclusão como fator etiológico das disfunções temporomandibulares (DTMs) tem sido um assunto polêmico e ainda controverso. Objetivo: Avaliar a correlação entre sinais e sintomas da disfunção temporomandibular e a severidade da má oclusão. Método: Foram avaliados 135 estudantes de Odontologia da UFPB. A presença de DTM foi estimada através do Índice Anamnésico de Fonseca (DMF) e de questões objetivas sobre seus sintomas. Os estudantes também foram submetidos a um protocolo resumido de avaliação clínica de DTM. A avaliação dos fatores oclusais foi realizada através do Índice de Prioridade de Tratamento (IPT) aplicado a modelos de gesso dos arcos dentários superior e inferior. As diferenças entre as médias do IPT relacionadas aos sinais e sintomas de DTM foram determinadas por meio dos testes t e One-way ANOVA. As correlações entre os fatores oclusais e a DTM foram determinadas a partir de correlação de Pearson. Resultado: A severidade da má oclusão, segundo o IPT, não influenciou no surgimento de DTM e de sinais clínicos musculares ou articulares, e na necessidade de tratamento. A má oclusão de classe II, trespasse vertical acentuado e dentes girados foram estatisticamente correlacionados à necessidade de tratamento e aos sinais clínicos de DTM. Conclusão: Em modelos multifatoriais, como na fisiopatologia da DTM, a oclusão pode desempenhar um papel de cofator na predisposição ou perpetuar as diferentes formas de DTM, não devendo ser considerada fator principal. .


Introduction: The role of occlusion as an etiologic factor of temporomandibular disorders (TMD) has been polemic and still controversial. Objective: To evaluate the correlation between signs and symptoms of temporomandibular disorders and severity of malocclusion. Method: A total of 135 undergraduate dental students from Federal University of Paraíba were evaluated. The presence of TMD was estimated by Fonseca's Anamnestic Index and objective questions about symptoms were addressed. Students were also submitted to a summarized protocol for clinical evaluation of TMD. The evaluation of occlusal factors was performed using the Treatment Priority Index (TPI) applied to plaster casts corresponding to upper and lower dental arches. The differences between the means of IPT related to signs and symptoms of TMD were determined by t tests and One-way ANOVA. Correlations between occlusal factors and TMD were determined by Pearson correlation. Result: According to the TPI, the severity of the malocclusion did not influence the onset of TMD, muscle or joint clinical signs and need for treatment. Class II malocclusion, marked overbite and rotated teeth were statistically correlated with the need for treatment and clinical signs of TMD. Conclusion: In multifactor models as in the pathophysiology of TMD, occlusion can play a role as a co-factor in predisposing or perpetuating the different forms of TMD. As such, it should not be considered a major factor. .

AIM: To evaluate the microshear bond strength of 4 dental adhesive systems after 1 year-water storage. MATERIAL AND METHODS: The sample consisted of 120 cylinders of composite, obtained from 24 bovine incisors, which were divided into four experimental groups: G1-Scotchbond Multi-Purpose, G2-Single Bond 2, G3-Clearfil SE Bond, G4-Adper Easy One and two storage times in distilled water: T0 - 24h and T1 - 1 year. A bivariate analysis was performed using the ANOVA and Tukey test (α=0.05). Results were evaluated inter-and intra-group in both times. RESULTS: After 24 hours, G1T0 (39.68A ± 11.55) showed higher bond strength (MPa) followed by G2T0 (22.71B ± 4.07), G3T0 (18.94BC ± 7.29) and G4T0 (13.30C ± 3.94). After 1 year of storage only G1T1 (33.95A ± 6.35) and G3T1 (13.59BC ± 2.63) maintained the bond strength values (p ≥ 0.05), while G2T1 (10.62C ± 4.32) and G4T1 (4.49D ± 2.49) presented a decrease in this values. CONCLUSION: The hydrophobic two-step adhesive systems maintained the bond strength after 1-year water storage.

BACKGROUND AND OBJECTIVES: Temporomandibular disorders are highly prevalent and may impair several oral function- related aspects. This study aimed at evaluating the impact of the presence and severity of temporomandibular disorder signs and symptoms on oral health-related quality of life. METHODS: Participated in the study 135 dentistry students of the Federal University of Paraiba. The presence of temporomandibular disorder was determined by means of an anamnesis questionnaire and a summarized clinical evaluation protocol. Oral health-related quality of life was determined by the summarized Oral Health Impact Profile version translated and validated for the Portuguese language. Statistical comparisons between Oral Health Impact Profile-14 means related to the presence of temporomandibular disorder signs and symptoms were achieved with Mann-Whitney and Kruskal-Wallis tests. RESULTS: Volunteers with temporomandibular disorder (p<0.001), needing treatment (p<0.001) and higher severity (p<0.001) had higher impact on oral health-related quality of life. Volunteers with clinical temporomandibular disorder signs had further quality of life impairment, being that individuals with simultaneous muscle and joint temporomandibular disorders (p=0.034) had higher Oral Health Impact Profile-14 scores. Most impaired domains were physical pain (p=0.045), functional limitation (p=0.007) and psychological discomfort (p=0.045). CONCLUSION: Temporomandibular disorder severity has negative impact on quality of life, especially in volunteers with simultaneous joint and muscle clinical signs. .


JUSTIFICATIVA E OBJETIVOS: As disfunções temporomandibulares são altamente prevalentes e podem comprometer diversos aspectos relacionados à função oral. O objetivo deste estudo foi avaliar o impacto da presença e gravidade dos sinais e sintomas de disfunção temporomandibular na qualidade de vida relacionada com a saúde oral. MÉTODOS: Cento e trinta e cinco estudantes de odontologia da Universidade Federal da Paraíba foram avaliados. A presença de disfunção temporomandibular foi determinada por meio de questionário anamnésico e por um protocolo resumido de avaliação clínica. A qualidade de vida relacionada com a saúde oral foi determinada por meio da versão resumida do Oral Health Impact Profile em sua versão traduzida e validada para o português. Comparações estatísticas entre as médias do Oral Health Impact Profile-14 relacionadas à presença de sinais e sintomas de disfunções temporomandibulares foram realizadas por meio dos testes de Mann-Whitney e Kruskal-Wallis. RESULTADOS: Os voluntários com disfunção temporomandibular (p<0,001), necessidade de tratamento (p<0,001) e maior gravidade (p<0,001) exibiram maior impacto na qualidade de vida relacionada com a saúde oral. Os voluntários com sinais clínicos de disfunção temporomandibular apresentaram maior comprometimento da qualidade de vida, sendo que os indivíduos com sinais de disfunção temporomandibular muscular e articular simultaneamente (p=0,034) apresentaram os maiores escores do Oral Health Impact Profile-14. Os domínios mais comprometidos foram dor física (p=0,045), limitação funcional (p=0,007) e desconforto psicológico (p=0,045). CONCLUSÃO: A gravidade da disfunção temporomandibular representa impacto negativo na qualidade de vida, especialmente em voluntários com sinais clínicos articulares e musculares simultâneos. .

Nos casos clínicos em que se verifica a diminuiçãoda Dimensão Vertical de Oclusão (DVO) e o desequilíbrio dosistema estomatognático, vários estudos sugerem areabilitação oral provisória com Próteses Parciais Removíveis(PPR) de recobrimento oclusal (tipo Overlay), associadas ounão com a confecção de restaurações, de modo a recuperara DVO, restabelecer o plano oclusal e eliminar alteraçõesenvolvidas com o desequilíbrio da oclusão, servindo comoguia orientadora da reabilitação final. Relato de caso: Esteartigo descreve o uso de PPR provisória tipo Overlay nareabilitação oral de paciente do sexo feminino, 56 anos, comcolapso oclusal posterior e desgastes dentários excessivos.Essa opção de abordagem foi adotada com o objetivo depreparar o meio bucal e reestabelecer a DVO previamente àreabilitação com próteses definitivas. A sequência detratamento envolveu os procedimentos de exame clínico eradiográfico, moldagem, determinação estética e funcionaldos planos oclusais, através do registro da DVO e da RelaçãoCêntrica (RC), montagem em Articulador Semi-Ajustável(ASA), provas clínicas e instalação/ajustes. Conclusão: Ainstalação da PPR provisória do tipo overlay contribuiu paraproteção e condicionamento dos tecidos orais, reestabelecimentosdas relações intermaxilares, e favorecimentoda estética, fonética e função mastigatória. Several studies have shown that in clinicalcases in which there is a decrease in the Occlusal VerticalDimension (OVD) and imbalance of the stomatognathicsystem, oral rehabilitation can be performed with the use ofRemovable Partial Dentures (RPD) with occlusal coverage(overlay type), associated or not with restorations, in orderto reestablish the OVD and occlusal plane and to eliminateocclusion alterations, thus functioning as a guide to finalrehabilitation. Clinical case report: This article describes theuse of provisional overlay RPD in oral rehabilitation of a femalepatient, 56 years old, with posterior occlusal collapse andexcessive tooth wear. This approach was adopted in orderto prepare the oral environment and restore the OVD prior torehabilitation with final prostheses. The sequence of treatmentinvolved clinical and radiographic examination procedures,tooth casting, aesthetic and functional reestablishment ofocclusal planes through the determination of OVD and CentricRelation (CR), cast-mounting in semi-adjustable articulator,prosthesis try-in and insertion, and follow-up. AIM: Conducting a systematic review of randomized clinical trials focusing on the efficacy of LLLT on pain control in patients with TMD, diagnosed by the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). MATERIAL AND METHODS: Search was performed at PubMed/MEDLINE database with the terms: (1) "Laser AND temporomandibular disorders"; (2) "Laser AND temporomandibular disorders AND RDC/TMD"; (3) "Low-level laser therapy AND temporomandibular disorders"; (4) "Low-level laser therapy AND temporomandibular disorders AND RDC/TMD"; (5) "Low-level laser therapy AND temporomandibular joint dysfuntion syndrome"; (6) "Low-level laser therapy AND temporomandibular joint dysfuntion syndrome AND RDC/TMD"; (7) "Laser AND temporomandibular joint dysfuntion syndrome" (8) "Laser AND RDC/TMD"; (9) "Low-level laser therapy AND RDC/TMD". Inclusion criteria: articles need to be randomized clinical trial performed in humans; evaluate the effect of LLLT in the treatment of TMD diagnosed by the use of RDC/TMD; published in English or Portuguese in the last 10 years. Protocol studies and pilot studies were excluded. RESULTS: Ten studies were included. The type of laser used was Gallium Aluminum Arsenide (GaAlAs) diode, with exception of 1 paper, which used super pulsed Gallium Arsenide laser. Eight studies reported decreased in pain levels, in two articles there was no statistically significant difference between test and placebo groups. CONCLUSION: In most studies, LLLT was effective in pain remission, but there is no standardization in parameters like wavelength, output power and frequency. Studies with more complex experimental designs, standardized diagnostic criteria for TMD and defined protocols for the use of LLLT are needed to determine its efficacy in the treatment of TMD.
